Parent Acknowledgment
As a parent or legal guardian, you acknowledge that your child is enrolling in an academy that uses Academy OS to manage training, attendance, payments, and communication.
Consent for Player Profile Creation
You consent to the academy creating a player profile that may include your child's name, date of birth, team assignment, attendance, and coach progress notes.
Consent for Training & Session Management
You consent to your child participating in scheduled training sessions and to attendance being recorded by academy staff.
Optional Media / Photo / Video Consent
Academies may take photos or videos during training, matches, and events for internal record keeping, parent updates, or promotional use. You may opt in or out of media use at any time by contacting your academy. (Detailed media consent flows will be added in a future onboarding update.)
Communication Consent
You consent to receiving service messages from your academy through Academy OS, including schedule updates, attendance notices, invoices, and announcements.
Emergency Contact Responsibility
You agree to provide accurate emergency contact information and to keep it up to date. Academy OS and the academy are not responsible for outcomes arising from outdated or incorrect emergency contact details.
Onboarding Checkboxes (Coming Soon)
The following consent checkboxes will appear during parent onboarding once that flow is integrated:
- I consent to my child's player profile being created on Academy OS.
- I consent to attendance and training session records being kept.
- I consent to receiving academy communications via Academy OS.
- (Optional) I consent to media/photo/video use for academy purposes.
